“Reconciling those two stories,” Rotondo explains, “is the core of my work as a clinical neuropsychologist.” It is also, in a sense, where his career began. Long before he studied the brain, Rotondo studied English literature, captivated by novelists such as Steinbeck and poets such as William Blake. He noticed something the rest of his classmates may have missed.

Novels, in their own way, are case studies. Every character is a portrait of human behavior. Every writer reveals something of their own psychology through style and voice. That observation carried Rotondo from literature to philosophy, then to psychology, and finally to neuropsychology. He describes the field as offering the same insight into people that literature does, only grounded in science.

Today, Rotondo evaluates patients across the age spectrum, testing cognitive functions such as memory, processing speed, executive functioning and attention. He also assesses psychological and psychiatric factors that shape how people think, feel and behave. His reports often travel further than his voice. By his own estimate, roughly 80 percent of the professionals who read his evaluations, including neurologists, therapists and teachers, will never speak with him directly. The clarity of his writing must do the work instead, a debt he traces directly back to his English degree.

Rotondo is candid about the parts of the job that are difficult to quantify. “Patients frequently underestimate their own cognitive standing,” he shares, “Many fixate on an average IQ score as though it were a verdict, rather than one data point among many.” He points out that intelligence takes forms testing cannot yet measure, from athletic skill to artistic talent. An artist of Pablo Picasso’s caliber, he notes, would not necessarily register as gifted on a standard IQ test. Likewise, older adults often assume normal aging means decline. And people struggling with depression may perceive their abilities as far lower than they actually are.

He is equally open about the limits of medication, preferring to see patients try therapy first when depression is not accompanied by harmful behaviors. For Rotondo, environment, not just brain chemistry, shapes mental health. “A prescription pad,” he explains, “is not always the first or only answer.”

That does not mean he discounts brain science, describing neuroplasticity as central to his work. Neuroplasticity is the brain’s capacity to change and strengthen connections through repetition, therapy, sleep and stress reduction. He has watched patients regain speech after strokes. He has also used testing results to help patients with substance use disorders recognize impairments they had denied for years. “This recognition,” he says, “has sometimes prompted a turn toward treatment that had eluded them.”

He is similarly thoughtful about AI’s growing role in mental health. Rotondo says he has tested chatbots by asking whether they “like” him. His goal was not curiosity about the software, but insight into how people form attachments to it. He believes AI can never be entirely neutral, because it is built from human input. Still, he sees potential for it to support therapy. It could offer guidance on exercise or mindfulness, for instance, without replacing human relationships.
